Change Request Form — Household

Need to update your household’s profile? Use this form when one or more members’ allergies, preferences, or serving needs change — or if you’d like to update your contact details. We’ll confirm your request by email and update the Household Profile section of your account as well as each individual profile where applicable.

Important: Changes for the upcoming week must be submitted by Thursday at 9 pm CST to be applied.

Membership Name

Name of the account holder (usually the person who initially applied for The Chef Edit).

Please list your household’s total servings per week for each meal category (not just the changes). Example: If you currently recieve 12 lunches, but want 2 more, you will write “14 total” in "Lunches" below. Additionally, if your household has individual profiles, please list the breakdown as well. Using the same example above, you will also need to specify: “14 total — 4 John, 3 Kate, 7 Laura”
